You can download the appropriate packet to have completed before coming to the Registrar's Office. There may be additional forms depending on the school your child is entering.

If you are living with a friend or family member (you are not listed on the lease/deed) please complete the
Families In Transition Form »

The School will determine start date once the registration process is complete.

A child currently in a Special Education program will start pending Child Study Team placement.

Previous school to provide when you unenroll the child:

• Transfer Card
• Report Card / Transcript
• Other pertinent school records

Parent/Guardian to provide:

• Original Birth Certificate or Affidavit Birth Certificate
• Immunization Record (must be reviewed by school nurse before child is admitted to school)
• Proof of Physical (if required)
• Legal documents for guardianship, parental custody or restricted contact (if applicable)
• Proof of Residency - Two (2) proof of residence documents
  • One must be one of the following: Tax Bill / Deed / Mortgage / Contract / Lease Contract / Letter from Landlord / Acceptance Letter from Park Manager (Harding Woods, Holly Tree Acres, Picnic Grove-Tullertown or The Villages I)
• If residing with family or friend (you are not listed on the lease/deed) complete the: Families In Transition Form »
• Elmer resident must have acceptance letter from Elmer School

Tuition [cut-off July 1] – any questions, please call Board Office 856-358-3094 ext. 4016 (Board of Education approval required)
